Care Finder delivers a face-to-face network of assistance to older vulnerable adults that helps to navigate through, and access aged care. Care finders helps individuals understand and access elder care and connects them to related community assistance.
Through Care Finder, our team makes your care journey easier by connecting you with providers to get the right care you need.
Why Care Finder?
Some older Australians require more assistance to navigate the aged care system, particularly those from culturally and linguistically diverse backgrounds. They may face:
- language and communication barriers
- information processing challenges brought on by cognitive decline
- unwillingness to engage with a need for support or government services.
Care Finder aims to improve the outcomes for eligible candidates by:
- improving the coordination of support when seeking access to aged care
- receiving a more thorough understanding of the aged care services and its access
- increasing the rate of connectivity to aged care services for those that are most vulnerable.
Who can access Care Finder?
Multicultural Australia's delivery of Care Finder is to those:
- over 65 years old
- living in Brisbane South
- from culturally and linguistically diverse backgrounds
- with limited support to access and navigate aged care services via MyAgedCare
